Abstract

Case history of a 20-year-old woman subjected to criminal abortion at 14 weeks of gestation is presented. The patient was admitted in a state of shock with manifestations of diffuse peritonitis. Preoperative diagnosis was of criminal septic abortion, uterine perforation, injury of the intestine, and diffuse peritonitis. Laparotomy indicated enlarged uterus, a 10x4 cm perforation of the posterior surface of the uterus opening into the Douglas space, absence of the rectosigmoid segment of the large intestine, absence of the cupula of the cecum, and absence of a 20-cm segment of the ileum. The patient underwent extirpation of the uterus, suturing of the vaginal stump, suturing of the rectum, sigmoidostomy, right hemicolectomy, and formation of the side-to-side ileotransversoanastomosis. Postoperatively, the patient received blood transfusions, and infusions of glucose and electrolyte solutions. Postoperative period was complicated by suppurative paraproctitis treated with antibiotics. The patient was discharged on day 37 postoperatively in a satisfactory condition.
