Abstract

94 patients with a mean age of 35 years (range 15-55, M76, F18) had undergone living related transplantation in Dhaka, Bangladesh and outside Bangladesh from 1980-1990 and followed up in Dhaka, Bangladesh for a mean period of 40 months (6 m to 10 yrs.). Of them 68 living related (LRT) and 26 were distant or unrelated (LURT) transplantatien. Transplantation were done in at least one haplo-identical match in donor related and blood group matching in unrelated or distant related transplantation. All patients had three to five units of blood transfusion and four patients had HbsAg positive before being transplanted. 70 patients received Aza-prednisolone regimen throughout the course and other 24 patients received Aza-prednisolone-cyclosporine regimen for first three months and then continued only Aza prednisolene therapy. The actuarial graft survival were 96% in the 1st yr. and 81% in 3 yrs. The graft survival were significantly better in the LRT group than LURT group. The frequent complications were tuberculosis (8.5%), chronic vascular rejecticn (5%), steroid induced diabetes (4%), persistant UTI (2%), avascular necrosis of bone (2%), cataract (1%). 26 patients died during this period. The known causes of death were severe renal failure (5%), severe infection (3%), stroke (3%), intestinal obstruction (4%), bleeding peptic ulcer (2%), tuberculosis (2%), viral hepatitis (2%), suicide (1%). LRT is the preferred method of treatment in patients with ESRD in Bangladesh.
