Abstract

INTRODUCTION: Suicide attempts and poisoning in pregnancy are a challenge for health professionals due to the unknown effects of the toxic agent antidote therapy and fetuses, and that those agents may have different behavior derived from physiological changes of pregnancy that make it more susceptible to certain toxic or drugs and their specific actions on the welfare of both mother and fetus. Case report: Female patient, 26 years old, 20 weeks pregnant, uncontrolled self-injurious purposes who ingested 20. g of aldicarb (carbamate), with subsequent presentation of predominantly epigastric abdominal pain, nausea, dyspnea, and then sialorrhea drowsiness, severe bradycardia and hypotension, which received management with fluid, gastric lavage, a dose of atropine and monitoring in intensive care for 24. hours, with satisfactory clinical outcome and high 72. hours following admission, adequate evolution of gestation.

DISCUSSION: Carbamate poisoning symptoms are derived from a reversible inhibition acetylcholinesterases, which gives rise to muscarinic and nicotinic symptoms of short duration, in this type of poisoning the fetus deserves the best care and ultrasound monitoring while the mother receives treatment similar to non-pregnant women because there is no evidence to contraindicate the use of the same scheme of detoxification and supportive therapy in pregnant women. No other case reported in the literature of acute carbamate during pregnancy, reason why the report itself. © 2014 Asociación Colombiana de Medicina Crítica y Cuidado lntensivo.
