Abstract

During adolescence, depression is not rare but it is difficult to recognize it, its clinical manifestations are less obvious than for adults. Nevertheless the depressive affects should not be trivialized and considered part of normal evolution. The two essential approaches in understanding depressive phenomena are the clinical aspect, research of symptoms, and the psychopathological point of view, trying to find the meaning of this episode in the psychic economy of the current adolescent. Adolescence is a crucial time in the internalization process of links with parents and organization of internal psychic space, with the resumption of differentiation between the oedipal superego and ego ideal. The case of Remi, from a semiological point of view, seems pretty poor, being a depression called "hidden" He was proposed a semi-structured diagnostic assessment with the use of the Kiddie SADS, and self-administered questionnaires: reasons for living inventory. Despite a relatively poor rating, a number of elements emerge and reveals to be interesting. Although the use of such tools is productive, in matter to achieve a better understanding of this suicide attempt, we tried to clarify the mechanisms involved in the psychopathological point of view. Although depressive symptoms may not be at the front rank of the clinical picture, there is such a suffering in Remi, who prevails in a certain narcissistic fragility. We assumed that the failure to his drivers licence highlighted the conflict that takes place between his ego and his ego ideal, functioning as a tyrannical authority. Remi started a psychotherapeutic follow-up which, while being meticulous on the diagnostic, must be creative in the meeting. The idea is that the subject by telling his story leads to his own discovery during the interview with the pleasure to tell about oneself. © 2011 Elsevier Masson SAS.
