Abstract

Elder Services, a division of Spokane Mental Health, focuses on social work case management of adults 60 years of age and greater. The goals of Elder Services are to prevent premature placement to the hospital, psychiatric ward, or long-term-care facility and to prevent suicide in this vulnerable community-dwelling population who often lack social and financial support. As a pharmacist working in this unique setting, the author describes how she receives referrals from social workers to assist with medication management, and provides medication education to staff, clients, their caregivers and families, pharmacy students, and the community.
