Abstract

Acute exogenous poisoning takes a significant part in child morbidity. Toxic influence on kidneys is relatively often. Renal structures and functions are impaired by direct toxic action or different system reactions as every renal structure can be affected. A lot of medicines and substances can provoke renal injury of tubular, glomerular, interstitial, vascular and obstructive level. The purpose of the investigation is to determine the frequency and clarify the characteristics of renal impairment in acute exogenous poisoning in childhood. 119 children with acute poisoning were admitted in Clinics of Pediatrics of University Hospital, Stara Zagora, Bulgaria, for a period of ten years (1995-2004). Renal impairment was found in 19 children (15,9%).Drug poisoning with renal disorders was found in 9 of them - suicide attempts or accidental poisoning (non-steroid anti-inflammation drugs, anti-epileptic drugs and anti-depressant).In the other 10 cases renal impairment is a result of poisoning with antifreeze, pesticides, snake-bite, as well as phalloid poisoning with lethal outcome. The toxic renal injury in drug intoxications could be: 1. Prerenal renal failure in 7 children (non-steroid anti-inflammation drugs, Theotard). 2. Acute tubular necrosis (ATN) and acute tubulointerstitial nephritis (ATIN) - of 2 children intoxicated with antiepileptic drugs. ATN is a frequent type of renal injury in the second group of intoxicatoins with toxic substance (antifreeze and phalloid mushrooms). Permanent renal injuries are not registered when following the renal function. Renal impairment in most cases is by dose- dependent mechanism.The relatively low percent of renal injury could be attributed to the timely diagnosis and treatment.
