Abstract

Consultation is requested on a 47-year-old white female with a long history of treatment-resistant bipolar disorder. The patient has a 27-year history of frequent hospitalizations for mania and psychosis, 3 previous serious suicide attempts, and low levels of functioning for the entire period. She has been cared for throughout her history by her parents and has always lived of home. Her parents are now in their 80s, and they and the patient are concerned regarding her inability to function independently and what the future will hold for her. She has also gained almost 100 pounds in the course of her illness and is very concerned about her health. Their hope is that a new treatment regimen, in particular newer medications, will make her less symptomatic and more functional, thus she will be able to live independently and perhaps even support herself.
