Abstract

Two cases of intoxication with opioid-like analgetics are reported and their significance for the occurrence of death is examined. Case 1: A 60-year-old alcoholic ingested 10 ml of Valoron N liquid (= 700 mg tilidine) and several tablets of Noctacepam (each containing 10 mg oxazepam) dissolved in beer to commit suicide. Even considering mild head injuries and an ischemic heart disease the results of the toxicological analysis (BAC of 1.4‰, blood: 0.7 mg/l oxazepam and 1.5 ml/l tilidine) left no doubt that the intoxication was the cause of death. Based upon the distribution of the drug and it's metabolites, acute (suicidal) intake can be differentiated from chronic administration in pain patients. Case 2: A 31-year-old female was given 2 x 0.4 g tramadol under intensive care including artificial respiration. Despite a toxic concentration of 5.6 mg/kg tramadol in blood, septic shock was established as the cause of death taking into account the clinical symptoms and pathology findings.
