Abstract

We analyzed the direct causes and background factors of recent serious chemical plant accidents. Behind the accident are common safety management issues such as inadequate equipment design, risk assessment, and change management. There are two aspects to the safety of chemical plants: "Process Safety" and "Personal Safety". In order to prevent serious accidents, it is necessary to predict serious accident scenarios from the design stage from the viewpoint of "Process Safety" and take risk reduction measures. At the manufacturing site, there are three major changes: "people," "equipment," and "expansion of control range." Skilled technicians have retired, and the generation that will operate the plant from now on has only the experience of operating an automated plant. The age of the equipment is 40 to 50 years, and the number of leak accidents due to aging is increasing. On the management side, the workload of each person, especially the workload of managers, is overflowing. In the future, it is necessary to accept this change and make up for the decline in human capacity with equipment and systems, and shift to risk-based safety management with an emphasis on preventing serious accidents.
