Abstract

In this paper, the author examines some examples that illustrate how forensic engineering investigation plays an important role in reducing risk and improving technology in great and small products. This paper gives an overview of the interrelationship of who is responsible for quality engineered systems, safety, and loss prevention, the organizational areas of responsibility, the causal factors. Next, the paper examines hazards that result from system failures, how to determine the probability of hazards occurring, the consequences of the hazards, and how to determine risks to personnel and loss prevention.
