Abstract

The death of any child from abuse or neglect is a tragic event that, rightly, rouses public indignation. Politicians and the media cry out for accountability, seeking someone or something to blame for what went wrong. Sadly, this culture of 'blame and failure' (Parton, 2016, p. 13) ignores the reality that 'for many of the children [who are the subject of Serious Case Reviews], the harms they suffered occurred in spite of all the work that professionals were doing to support and protect them' (Sidebotham et al., 2016, p. 12) and that the professionals themselves are often those who are most committed to reflection, learning and improvement...
