Abstract

Through the survey, most of accidents mainly resulted from human errors in the petroleum operation and literature mainly focuses on the analysis and evaluation of human errors (HEs). So, based on the risk management theory, the study develops: (a) the probability model of HE of the single key operation for the operator, (b) the probability model of the accident occurrence for a equipment system, (c) the accident risk assessment models of HEs for a equipment system and an organization.
