Abstract
This paper discusses the causes and reconstructs the dynamics of the catastrophic accident occurred at the BP refinery of Texas City in 2005. Dedicated sections describe the hypotheses and models adopted in the analysis. Inconsistencies and omitted points in existing reports are also examined.
