Abstract

The violence of child and adolescent that occurs in specialized institution, in school environment or at home, represents a frequent motive for consultation or hospitalization. Hospital of the Children of Toulouse, which admits 33,000 urgencies a year, is the only establishment reception of urgency of the department. Nevertheless we notice that our psychiatric teams are very rarely confronted with violent act out. We wonder about the reasons of this apparent quietness and possible function para-exciting of the paediatric device: are the density and the quality of the network of care upstream sufficient? Does age of recruitment (before 15 and a half years) protect the most noisy demonstrations? We shall tempt to analyse the dynamics at stake up to the paediatric hospitalization in its relation with the expression of the aggressive drives. We shall put in evidence successively the role of the physical mediation, the organized regression, the narcissistic overinvestment, absence of constraints and of "against push". Furthermore, we notice that when a violent act auto or hereto-aggressive arises in Hospital of the Children, capacities stamina seem quickly exceeded and an orientation should be envisaged as soon as possible. Since more one year, a local network of psychic care sets up intended for the adolescents integrating Hospital of the Children. This new organization makes indispensable better to understand mechanisms, capacities and limits of each of the teams to create common representations and clarify the indications.
