Abstract

This paper presents an attempt to derive a technique for predicting hazardous situations, from a simple information flow model of human skilled performance. The technique is based on the assumption that any breakdown in the flow of information through the system is potentially hazardous, and an analysis chart is presented which attempts to formalise the search for these breakdowns.
