SP - 26 EP - 33 VL - 41 IS - 1 N2 - We report about the rare combination of an CO-intoxication, a severe cerebral edema and a spontaneus mediastinal emphysema in an 18-year-old man due to an suicide attempt. Caused by the fast decrease of the CO-Hb-concentration we decided to treat the patient with a conservative invasive ventilation strategy with high-flow oxygen concentration. Within the next 24 hours, the patient developed a further increase of the mediastinal emphysema with need for a mediastinoscopy and development of an adult respiratory distress syndrome due to the initial aspiration syndrome. On day 8, the patient was successfully weaned from the ventilator and was transferred to a rehabilitation facility. © 2016 Dustri-Verlag Dr. Karl Feistle.
