Cardiac complications are observed due to loss of cardiac mass and initially manifests with bradycardia. We reported the case of a 15-year-old boy who presented with AN and severe bradycardia. Electrocardiogram showed sinus bradycardia with first-degree atrioventricular block and noted several episodes of bradycardia from 24-hours holter monitoring. With a multidisciplinary team management and improvement of his nutritional status and body weight, his bradycardia resolved.
