%X <p>[SafetyLit note: Chinese rocks is a slang term for certain traditional remedies (as in this case) or drugs of abuse -- usually an opioid. Probably the term is from:  "China white" heroin.]   Toads have been used for ritual and medicinal purposes since ancient times, particularly in Asian and Mesoamerican countries, but the skin, parotid glands and eggs of certain species contain alkaloids and cardioactive steroids1 that can produce a digitalis-like poisoning. In addition, there is a preparation in the form of dry extract obtained from the toad secretions that is used for sexual purposes. This extract, called love stone, Chinese stones, or Jamaican stone, is applied on the ventral side of the glans, producing an anesthetic effect that delays ejaculation. It is a resinous substance, approximately 1 cm2. After applying they should wash their hands and avoid oral sex. We present the first case reported in Spain of lethal poisoning with suicidal intent by this substance  A 23-year-old woman who went to a primary care center for nausea, vomiting, anxiety and agitation following the intake of a substance called Chinese rocks with suicidal ideation three hours earlier. PA 153/70 mmHg, HR 55 bpm, baseline oxygen saturation 99%, normal body temperature and capillary blood glucose 122 mg / dl were found. An electrocardiogram showed sinus bradycardia at 50 bpm. It was decided to transfer by conventional ambulance to our center, during which there was a serious deterioration, and it came with signs of poor perfusion and preagonic breathing. The first recorded rhythm was asystole, beginning advanced cardiopulmonary resuscitation maneuvers, which lasted for 60 minutes, without achieving its own rhythm. In the analysis K highlighted 7.76 mmol / l (VN: 3.5-5.1 mmol / l). Digoxin determination was not requested. Judicial necropsy was performed, which revealed widespread congestion, edema and diffuse pulmonary hemorrhage. The microscopic findings confirmed these findings. The presence of digoxin was detected by enzyme immunoassay (Indiko Plus, Thermo Scientific, DRI®) in concentrations of 0.004 mg / l. Caffeine and paracetamol were also detected in blood and tetracaine in bladder lavage by chromatographic techniques.
