abstract="The aim of this case report is to show the efficacy of the treatment with olanzapine in a patient suffering from organic affective disorder who attempted suicide, and to initiate discussion about broadening the indications to this diagnostic category, to treat it with an atypical antipsychotic such as olanzapine. The patient was treated on three, occasion for organic affective disorder due to a demyeliniating disease of vascular etiology. On all three hospitalizations, the clinical picture was predominated by depression with psychotic elements and distinct suicidal tendencies. On the first two hospitalizations, the patient was treated with typical antipsychotics and antidepressants. The third hospitalization was preceded by a suicide attempt (pills), when olanzapine was introduced into treatment and the patient responded well to it. Delusional depressive phenomenology was successfully treated, with mood improvement, better cooperation, and thus a lower risk of suicide. The choice of olanzapine was based on by positive previous experience, good treatment of psychosis, improvement of depressiveness, and a lower risk of suicide in other diagnostic categories, in the first place among patients suffering from schizophrenia with depressive symptoms. There are not many scientific papers dealing with the treatment of the organic psycho-syndrome with olanzapine, yet, some more data are available from papers reporting on the treatment of various forms of dementia with intramuscular olanzapine. The treatment of the patient with organic affective disorder with olanzapine proved very successful. This is supported by the longest remission achieved thus far. Similar results have also been recorded in other patients with organic psycho-syndrome of various etiologies treated with olanzapine. Additional studies of the therapeutic efficacy of olanzapine in this group of patients will hopefully confirm this report.<p /><p>Language: hr</p>",
