Abstract

Cautions

Despite being an advance in CAP research methodology, CAPNET has some meaningful cautions. The subjects enrolled in CAPNET are nested cohorts of children evaluated in (currently) 11 children's hospitals across the U.S. As noted by Kratchman and colleagues (2022) there will be site variability that will need to be accounted for in all data analysis. This variability could result in differences in rates of imaging or testing, or CPA prevalence rates, by site, which would need to be accounted for
Future evolution...

While CAPNET will advance our understanding of the care of child suspected victims of CPA by CAP providers, there remain some pressing needs in the research landscape. First, CAPNET will be valuable to describe the current care and findings of suspected victims of CPA, but it will be limited in being able to describe the best way to care for these children. This will require moving from observational to interventional research protocols. In the way that the conventional scientific process often
Remaining gaps...

There are two looming gaps that still require large, coordinated research infrastructures. First is that the "disease" of child maltreatment in the U.S. has changed over the past 3 decades. Starting in the late 1990s, the relative rates of CPA and child sexual abuse began to drop nationally. (Bullinger et al., 2019; Finkelhor et al., 2020) In 1995, 52 % of maltreatment was neglect with CPA representing 25 % of all substantiations.(U.S. Department of Health and Human Services, 1997) The most...
